Transitional Case Management

Helping ex-offenders transition to sober, crime-free lives in the community 

Weekly case management and drug/alcohol counseling begin in prison, and continue in the community. CRT helps participants meet basic needs such as acquiring housing, clothing, food, transportation and employment. Counseling, groups and drug testing for up to four months post-release. Staff speaks English and Spanish. Only referrals from DOC are accepted for this program.

To Apply
Referrals are only accepted from the DOC. Adult male inmates with a history of substance use re-entering Greater Hartford communities from prison are eligible.

Interested inmates must:

  • Attend an information session in prison
  • Get a referral from your DOC counselor
  • Write a letter of interest and submit to their DOC Counselor
  • Complete the Texas Christian University screening form
  • Complete an initial interview form
  • Interview for the program

Additional Details
Outpatient substance abuse treatment, individual counseling, sober housing, drug screening, intensive case management, and assistance meeting basic needs, such as clothing, transportation, housing, employment. Counselors if you would like to refer a client call the Program Coordinator at (860) 714 - 2932.
