Permanent Supportive Housing

 

Services

The CRT Permanent Supportive Housing Program provides financial assistance and supportive services to homeless families with children. Length of program is for five years, at which point families graduate and stay in an apartment or move to other permanent housing. Clients must have a disability and be motivated to go through a serious life-changing program that incorporates case management, home visits, behavioral health, training, and more.

 

Eligibility Requirements

Clients must be:
- Head of household with children
- Living in a homeless shelter, street or uninhabitable environment
- Have a diagnosed disability
- Be eligible to put utilities in own name
- Have income and be able to pay fee of 30% of gross income

 

Application Process

Families are referred to the program by caseworkers from homeless shelters.
