DOC Project Transition Program

Services

The CRT DOC Project Transition Program provides temporary housing and supportive services to men and women who are being released from prison and would be homeless without intervention.

The program provides intensive case management services, clinical services, and vocational services. The program is for four months at which point clients graduate and move to other transitional or permanent housing.

Application Process

Clients are referred to the program from the Department of Correction parole liaison.

Eligibility Requirements

Clients must be under DOC transitional supervision, 18 years and older residing in Hartford, have a history of serving time in prison. Clients must be able to obtain employment and pay $50 of their income toward their program fee.
