Services

The CRT Transitional Case Management for Offenders Program provides intensive comprehensive case management services for Department of Correction offenders reentering the Hartford community.

Eligibility Requirements

Clients can either be male or female, 18 years of age or older, and must be Department of Corrections prisoner who will be released in 3 months.

Application Process

Clients must be referred to the program by a Department of Corrections caseworker.
