Eligibility RequirementsClients must be homeless individuals within the Hartford community. |
Application ProcessA homeless outreach team does intake in the field. Further assessment and services may be clinic based. |
Service Hours and LocationsAll area shelters or living on the street. 675 Tower Avenue, Suite 308 Monday - Friday
