Community Housing Assistance Program (CHAPs)

Services

The Community Housing Assistance Program is a supervised scattered site housing program for DCF Mandated Adolescents preparing for independent living. Case management services include: budgeting, life skills, parenting skills, educational assistance, work readiness and job retention skills. These youth will age out of the foster system without a permanent family structure.

Eligibility Requirements

- DCF committed adolescents referred by their DCF Social Worker
- DCF Adolescents who have successfully completed or are enrolled in an accredited Community Life Skill Program
- Must be between the ages of 16-21
- Must be enrolled in a full-time educational program

Service Hours and Locations

Services are available 24 hours per day 7 days per week. Scattered site apartments managed at:

Application Process

Must be referred by a DCF Social Worker.
